Note: If you believe you have a medical emergency, call 911 or your physician immediately.

*Your completion of this Screening Form is your agreement to abide by VIP Medical Clinic Policy which will be emailed to you prior to your agreement and payment. Currently, Quest Diagnostics laboratories are deferring antibody blood test fees in anticipation that the government or insurance will ultimately compensate them, however they DO require a physician’s order to do this test.  If you do NOT have a physician or medical insurance, our service is specifically designed just for you.  The VIP Medical Clinic is a VIP Service for those who do NOT wish to wait HOURS in crowded Waiting Rooms–possibly exposed to infection–and who wish to get tested QUICKLY. In particular, for those without medical insurance or a doctor, our telemedicine service is a more convenient and safer option which is generally far more affordable that the Urgent Care or Emergency Room without draining their limited resources.  We are not unlike airline VIP Pre-Check Security to avoid WAITING in long screening lines in all airports.  Please note we always recommend you first confirm if your insurance provider will do testing at minimal cost to you–because you have already pre-paid for this service. Also certain hospitals or government agencies may offer limited free testing under government subsidy or grants, and you should first see if they have any availability, since our only compensation is payment through you. We will do our best to expedite your results back to you as soon as possible, but cannot promise how quickly because of the overwhelming volume anticipated. Our commitment is simply that we will all do our very best for you.

**The Office for Civil Rights (OCR) at the Department of Health and Human Services (HHS) have relaxed the Health Insurance Portability and Accountability Act of 1996 (HIPAA) patient privacy requirements, later amended by Health Information Technology for Economic and Clinical Health  (HITECH) Act to protect the privacy and security of protected health information due to the COVID-19 crisis to allow us to use ZOOM–which just announced enhanced security on 04/03/2020–for telemedicine.  Please note that by using our service, we cannot guarantee patient privacy and must disclaim all liability in our effort to provide this critical service quickly to you.  As we are able, we will enhance this technology, nevertheless your acceptance of our telemedicine service herein constitutes your acceptance of this agreement in this time of international COVID-19 crisis.
